Lifewell successfully transitions Arcadia, South Central and Midtown direct care clinics from CHOICES

Effective August 1, 2015 Lifewell Behavioral Wellness assumed operational responsibility for three of the CHOICES six clinics.

  •  Lifewell assumed responsibility for Arcadia, South Central and Midtown clinics
  • The other three CHOICES clinics (Enclave, Townley and West McDowell) transitioned to Terros.

The transition to Lifewell has gone smoothly and the clinics are serving members as usual. We greatly appreciate the patience and support of members and staff in making the transition a success. Lifewell held forums for members and families at each of the clinic sites during August to address any questions or concerns. We will continue to periodically hold forums in the future and have scheduled a forum at Arcadia for September 17th from 3pm to 4pm.
Lifewell also operates four outpatient centers throughout the valley which are focused on counseling and rehabilitation services. These sites are known as Lifewellness Centers. In keeping with our emphasis on integration and wellness, the direct care clinic sites will also be known as Lifewellness Centers. Contact information for the clinic sites can be found on the Lifewell website under the Contact Us tab or click on the Lifewell Service Hubs section.
We are excited and honored to assume responsibility for direct care clinic services at Arcadia, Midtown and South Central. We look forward to sharing the journey with members and staff for better health and wellness.

CHOICES to transition clinics to Lifewell and Terros

Effective July 1, 2015 Lifewell Behavioral Wellness and Terros will assume operational responsibility for the six CHOICES clinics.

  • Lifewell will assume responsibility for Arcadia, South Central and Midtown clinics
  • Terros will assume responsibility for Enclave, Townley and West McDowell clinics

This transition is administrative and will not affect member services.  Members were sent a letter from Mercy Maricopa Integrated Care informing them of the change and assuring them that their treatment teams, clinic locations and phone numbers, and services will not change.  Members will continue to receive services at the same location where they’re currently receiving services.  They also will receive assistance to transfer their medical records to Lifewell and Terros from CHOICES.

The agreement between CHOICES, Lifewell and Terros is a natural transition that reflects many years of collaboration and decades of work in Arizona’s behavioral health system.  Lifewell, Terros and CHOICES have been key partners since CHOICES launched in 2007.  Lifewell (formerly New Arizona Family and Triple R) and Terros were founding CHOICES members and have been on the CHOICES Board of Directors since its inception.  During the past year, both organizations have taken on increased management roles in CHOICES program operations.

Lifewell and Terros are long-standing, well-established providers of behavioral health services in Maricopa County.  Lifewell and Terros have delivered supportive services to members served by the CHOICES clinic sites for many years.  This shared history and familiarity with members, together with the behavioral health expertise of Lifewell and Terros, will result in a seamless transition for members.

Continuity of member care and staff retention is critically important and Lifewell is working closely with CHOICES and Terros to finalize the details of this transition.  Updates on the transition will be provided on the Lifewell website as we progress.

Welcome CHOICES members and staff
